I've always treated Doctors and Bodyguards as two separate roles. Doctors heal, Bodyguards take a shot for someone

If by take the bullet he means bodyguard die, then that's just how we both learned to play. It's the typical mafia deviations of those roles.

I do, too. The problem I have with the spec Jeff was spitting out was the no self protection rule.
