I can only think of 2 times when WWE broke storyline to announce a legit injury on the air as it happened. Owen Hart falling from the catwalks to his death in 1999. I remember Jim Ross repeatedly telling the TV audience "This is not storyline. This is not part of the payback show. This is real." And then Jerry Lawler having a heart attack on RAW 4 years ago. Michael Cole was caught completely off guard. He must have said "Um" every 4-5 seconds when he made the announcement.
