IMO that's a typical Cena hater response. People find anything to use against Cena as normally there isn't much (apart from stuff handed to him from creative).

If Cena did improvise then yeah, he fucked up bad. But I doubt WWE would include it in the constant recaps if it wasn't planned.

It's well known/documented that Orton was a douchebag. Apparently he has mellowed recently, but I've heard horror stories of him being an asshole to fans, shitting in divas purses, ect.
