ACW's Graphics Request Thread

Andy asked me to try my hand at this, so here's the request topic. If you want a ACW banner, just fill out the form below, and I'll post whatever I get within a few days.

Guidelines
For the name, I'm willing to put in nicknames, but the longer the name; the smaller the font!
For background color, you can basically choose any color or any combination of two colors.
To guarantee best results, it'd be great if your character's appearance was mainstream (I.E. famous enough that I could find a PSD for them). If not, I'll try cutting a specific picture but no guarantee.

If you want a specific font/PSD to be used. I linked some of the sites I use.
