Nope the chairmen is at the top of the hieracy. I don't think Triple H is actually the coo actually I think it's just in the storyline.

No idea. The 40 man was a flop I think.

Triple H is one behind Vince I think, so I assume he is COO. If he's not, he's still got more power than anyone else in the back room.
