I think it can only happen with Big+ sized companies, but basically the company suffers a plane crash where a majority of the roster dies.

I like to combine it... or make my own random events with dice rolls, just to get a bit more "realism" in it, if you will

TBF you can turn them off, injuries and deaths.

I've never seen it happen in a game but I do know it's existed as far back as TEW 2013.
